I have one for stills that you can edit. The basics are there, just drop me an email if you want one and I will send it over.

Another thread resurrected from the beyond, I'm glad people are starting to use the search function. Videomaker.com also sells a "book of forms" it covers pretty much everything including property/location release forms.
