pink and green lines in capture screen

when I capture video in prem pro 1.5 I have thick pink and green horizontal lines in the capture window...no footage...it captures correctly and I can control my camera..however there is no picture.....I have had the problem before but then it went away...I disconnect the camera and plug it back in but the lines come back.
