Default zooming from a photo

- i was watching a documenry on the shooting of john lennon a week or so ago and saw a really good effect that they used on a photo...

basicaly the camera started very close to the still image and it was in focus, it then slowly zoomed out and an object (a tree for example) came into the foreground. at this point the new object was in focus and the backround was blured.. it continued to zoom out until another object came into focus and everything that was behind it then went out of focus.. it continued like this until all of the image was in shot - and at this point the whole image was all in focus and looked crisp.. ..

any ideas on how this is done???
